A Case of Angiomatoid Epithelioid Sarcoma Mimicking an Epithelioid Vascular Neoplasm

Epithelioid sarcoma is a rare but aggressive soft tissue that presents significant diagnostic and management challenge to clinicians. We report 31-year-old female who presented with non-healing ulcer of three years’ duration on her left thumb after burn. Examination revealed an ulcerative tumour invading the bone areas haemorrhage. Microscopic examination showed epithelioid neoplasm central necrosis. The neoplastic cells formed dyscohesive sheets, slit-like channels, pseudovascular spaces. Erythrocytes were frequently observed in these areas. malignant diffusely positive for cytokeratin AE1/AE3, EMA, vimentin, Fli-1; while focally CD34 CD31. morphology immunophenotype overlap vascular neoplasm. Additional immunohistochemical stain loss SMARCB1/INI1 expression supporting diagnosis distal type, angiomatoid variant. present distinctive case developed burn highlight pathological characteristics distinguish it from neoplasms.
